If you are applying for a self-certification extension, you do not need to provide evidence. If you are applying for an evidenced extension this request will not be considered unless evidence is provided. You should provide as much supporting evidence as possible, which should be from an appropriate third party and time-relevant, relating to the circumstances you are applying for.
